  1. This medication will be shipped to you from SPRING CREEK PHARMACY.
  2. Injection supplies are included.
  3. The current shipping time is around 7-10 days, but often ships much sooner.
  4. It will arrive to you cool and will need to be kept in the refrigerator.
  5. Normal dosing for Triple G is 1mg, 2mg, 4mg, 8mg, or 12mg. Spring Creek ONLY OFFERS THE 8MG OR 12MG DOSE. These are the most common doses and the lower doses are rarely used since Triple G is reserved for patients that have not responded to the higher doses of TZ. If you need to order one of the smaller doses, please order from Striker Pharmacy HERE.
  6. The strength on your vial is 8mg/0.5ml (may be expressed as 16mg/ml). The 2ml vial will have 4 weeks of the 8mg dose and you will inject 50 units weekly. The 3ml vial will have 4 weeks of the 12mg dose and you will inject 75 units weekly. Please do not alter your dosing without discussing this with me.
  7. You MUST order the dosage you intend to take, as these vials are designed to be used for 4 weeks only. We will be very strict on this and will carefully monitor your ordering practices. To be successful, you should order every 3-4 weeks to make sure you don't miss any doses.
  8. You may order ONE VIAL at a time due to the BUDs on the vials.

WHAT IS THE TRIPLE G INJECTION?


If you keep up with the “latest and greatest” weight loss medications, you have probably heard of the revolutionary new drug currently in phase 3 of clinical trials on the path to FDA approval for treating obesity. We refer to it as the “Triple G” drug because of its action on three different receptors in your body that leads to weight loss: GLP-1, GIP, and GCG receptors.

  • GLP-1 (glucagon-like peptide-1) receptor agonists: Medications such as Saxenda (liraglutide) and Ozempic (sema) were the first of these types of medications. Wegovy (sema) is FDA-approved for weight loss. Compounded sema would fall in this category as well.
  • GLP-1 PLUS GIP (glucose-dependents insulinotropic polypeptide) receptor agonists: When Mounjaro (TZ) was introduced in 2022, it was clear that it’s dual-action properties lead to superior weight loss in studies. Zepbound (TZ), released in November 2023, is the FDA-approved version of Mounjaro. Compounded TZ would fall in this category as well,
  • GLP-1 PLUS GIP PLUS GCG (glucagon) receptor agonists: This brand-new medication is a “Triple G” in that it acts on three different receptors. This can have additional benefits on appetite control and fat burning.

While Triple G has quickly emerged as an exciting new weight-loss medication, it is important to know that it is NOT an FDA-approved medication yet. Phase 2 trials have been completed, however, and the results have been nothing but amazing. The study, published in the New England Journal of Medicine (NEJM), included 338 adults with obesity or overweight.

  • After 24 weeks, the people who received 12 milligrams of Triple G—the highest dosage—saw up to 17.5% weight loss. After 48 weeks, the group saw 24.2% average weight loss.
  • Every patient that received the highest dose (12mg) lost at least 5% of their body weight and about 1 in 4 patients lost 30% or more of their body weight!
  • Participants lost an average of about 60 pounds over an 11-month period.
  • Also, by the end of the study, the research suggested that participants hadn’t reached their weight plateau yet. That could mean even more weight loss might be possible when using Triple G over a longer period.
  • Although it has not been tested head-to-head against sema or TZ, weight loss in the trial seemed to happen quicker than other weight loss medications. In its initial studies, sem on average reduced body weight by around 15%, or about 34 pounds, after 68 weeks. TZ was shown to reduce body weight, by 22.5% on average, or about 52 pounds, after 72 weeks.
  • These early studies indicate that Triple G has a similar side effect and safety profile as tirzepatide and semaglutide.

Triple G Availability

As mentioned above, Triple G is still undergoing clinical trials before it can be FDA-approved. It is currently in phase 3, but it still could be a few years before it is available commercially as a brand name medication. This means that it will be a while before you can pick this up at your local pharmacy (like Wegovy or Zepbound).

However, some compounding pharmacies are now offering compounded Triple G. They are able to source this Triple G peptide from the same reputable sources where they obtain sema and TZ. I am now able to provide this medication to select patients.




Qualifications/Triple G protocol

Triple G is now available for all my patients if they fall into one of the two categories below (based on your CURRENT BMI):

1) CATEGORY 1

Patients with a CURRENT BMI of 40+ (or 35+ with risk factors such as high blood pressure, high cholesterol/lipids, or sleep apnea) may qualify to begin Triple G and would dose as follows:


  • If you have never been on TZ or sema, you would start at the 1mg starter dose for 4 weeks. I’d prefer you to use TZ first, but I will allow those with higher BMIs as above to start with Triple G or switch earlier than those with lower BMIs.

  • If you are currently on TZ and would like to switch to Triple G:

    • If you have recently completed the 2.5mg dose of TZ and feel it is not effective, order the 2mg dose of Triple G.
    • If you have recently completed the 5mg dose of tirzepatide and feel it is not effective, order the 4mg dose of Triple G.
    • If you have recently completed the 7.5mg dose of tirzepatide and feel it is not effective, order the 8mg dose of Triple G.
    • If you have recently completed the 10mg dose or higher of tirzepatide and feel it is not effective, order the 12mg dose of Triple G.

  • If you are currently on sema:

    • Patients are encouraged to order a trial period of TZ before ordering Triple G. Only patients in category one will be allowed to order Triple G prior to a trial period of TZ.
    • If you have recently completed the 0.25mg dose of sema and feel it is not effective, order the starter 1mg dose of Triple G.
    • If you have recently completed the 0.5mg dose of sema and feel it is not effective, order the 2mg dose of Triple G.
    • If you have recently completed the 1mg dose of sema and feel it is not effective, order the 4mg dose of Triple G.
    • If you have recently completed the 1.7mg or 2.4/2.5mg dose of sema and feel it is not effective, order the 8mg dose of Triple G.

      YOU MAY ALSO START AT A LOWER DOSE IF DESIRED. I ALWAYS RECOMMEND USING THE LOWEST DOSE POSSIBLE

2) CATEGORY TWO

Patients with a CURRENT BMI of 30-39 (or 27+ with risk factors of high blood pressure, high cholesterol/lipids, or sleep apnea) who have stalled on TZ therapy may qualify as follows to begin Triple G:

  • If you have never been on sema or TZ, you would not qualify for Triple G at this time.

  • If you are currently on TZ:

    • Patients that have stalled on higher doses of TZ may request approval to order Triple G. However, there must not be any recent gaps in your TZ use in order to assess that you have stalled.
    • You must have recently completed at least 4 consecutive weeks of the 10mg dose or higher of TZ. If you feel you have stalled and your weight loss had dropped below losing 1-2 pounds a week, you may request approval to order Triple G.
    • If you have not recently completed at least 4 consecutive weeks of the TZ 10mg dose or higher, you may not request Triple G until you have done so. Please message us for ordering guidelines.
    • Please use the same ordering guidelines as CATEGORY ONE patients above when choosing the Triple G dose for your first order.

  • If you are currently on sema:
    • You would switch to TZ before being approved to order Triple G.
    • You would then follow the above guidelines and request Triple G after stalling on the 10mg dose or higher of TZ.

      YOU MAY ALSO START AT A LOWER DOSE IF DESIRED. I ALWAYS RECOMMEND USING THE LOWEST DOSE POSSIBLE


PLEASE NOTE: These BMI requirements are starting requirements only, similar TZ and sema. You will still be eligible for the medication after your BMI falls below the initial criteria when you lose weight from using your medication.
