TZ/L-Carnitine/NAD+ SC INJECTION with E-Visit TX STAR PHARMACY

4. There are two different vial concentrations, a TZ 5mg/ml and a TZ 15mg/ml concentration. The concentration of NAD+ in all vials is 40mg/ml. The concentration of L-Carnitine is 250mg/ml in all vials.
5. The 2.5mg and the 5mg doses use the TZ 5mg/ml concentration vial. The four higher doses use the TZ 15mg/ml concentration vial.
6. Be sure to read the directions on your vial for proper dosing! There are two different vial concentrations, so the number of units you inject will depend on the package.
7. It will arrive to you cool and will need to be kept in the refrigerator.
8. You must order the dose you intend to take! If you need to alter your dose in any way, please discuss this with our team by reaching out through your Push Health account.

10. The BUD on your vial is 90 days from the date of compounding. Please see Joint Commission guidelines below (scroll down). You may order up to TWO packages at a time. For the 5mg dose, you would receive 4 vials. For all other doses you would receive two vials.
11. Note that the two highest doses use the same vial, which has 60mg. Therefore, I suggest injecting 12mg (80 units) instead of 12.5mg (83 units). Doing this allows for 5 doses instead of 4 from this vial. Please also see Joint Commission guidelines regarding this dosing.

DOSING: Some of these are odd units, so just get as close as you can on your syringe. Please note the unit change for the packages.

AVAILABLE PACKAGES (also see above dosing table):

1. 2.5mg X 4 weeks: Uses the 5mg/ml 2ml vial. You receive one vial. Your dose is 50 units.
2. 5mg X 4 weeks: Uses the same 5mg/ml 2ml vial as above. You receive two vials. Your dose is 100 units.
   
   
   1. This is a great package for those who are using the 5mg dose for maintenance and spreading your doses out to every 10-14 days, since your first two doses come in one vial, and your last two doses come in another vial. You will receive them both at the same time, but this allows your 3rd and 4th dose to remain in an unpunctured vial longer in your refrigerator.
   2. Unfortunately, you have to inject 100 units for the 5mg dose with this vial, which can be uncomfortable for some patients. If you are uncomfortable with this, please order a 5mg dose from another pharmacy.
3. 7.5mg X 4 weeks: Uses the 15mg/ml vial 2ml vial. Your dose is 50 units.
4. 10mg X 4 weeks: Uses the 15mg/ml 3ml vial. Your dose is 66 units.
5. 12mg* X 5 weeks: Uses the 15mg/ml 4ml vial. Your dose is 80 units.
   
   
   1. This vial has 60mg total. If you injected the 12.5mg X 4 weeks, you'd have 10mg left over. Alternatively, if you reduce your dose to 12mg, you get 5 weeks of that dose! Of course, make sure you read the JC Guidelines and the bottom if this page.
   2. The 12.5mg dose would be 83 units instead of 80 units (12mg dose).
6. 15mg X 4 weeks: Uses the same vial as above. Your dose is 100 units. Unfortunately, as with the 5mg dose with the other vial concentration, you have to inject 100 units for the 15mg dose with this vial, which can be uncomfortable for some patients. If you are uncomfortable with this, please order a 15mg dose from another pharmacy.

New patients not previously on an injectable weight loss medication will start at the 2.5mg for the first 4 weeks. You may then move up to the 5mg dose for the next 4 weeks, then 7.5mg for 4 weeks, etc. However, you do NOT have to move up in dosing until you are ready. If you are having successful weight loss on a certain dose, you do not have to move up. Also, if you are having significant side effects on a dose, you should spend another 4 weeks on your current dose before you move up.

Patients in the weight loss phase should be injecting weekly for best results!

Joint Commission guidelines for proper use of many multi-dose vials include the recommendation to discard the vial 4 weeks after first puncture, even though the Beyond Use Date (BUD) may be longer. These guidelines are generally followed in office and hospital settings, particularly when the vial is used for multiple different patients. However, this same suggestion is made for vials punctured several times a day or only once a week. With certain vial sizes/doses/packages, using all of the medication in the vial without wasting any could result in a patient using the vial for longer than 28 days after first puncture. If you choose to use your vial longer than 28 days after first puncture due to package size or maintenance dosing, you must acknowledge that you understand the Joint Commission recommendations to discard the vial 28 days after first puncture, even if you only puncture the vial once a week. You will be using the vial at your own discretion.
