SOME NOTES ABOUT THIS ORAL SUSPENSION:

• This is a daily dose taken under the tongue (as opposed to a weekly injection). This is a liquid medicine that should be help under the tongue for 1-2 minutes. Avoid food or drinks 30 minutes before and after your dose. Many patients prefer to take it at night before bed.
• The INJECTABLE forms of TZ is more effective than any oral form. The bioavailability versus the injectable form varies, and you likely will not see results as quickly as you do with the injectable form.
• However, these are an excellent alternative for people who do not want to use the injectable form of TZ. For those who have difficulty tolerating even the lowest doses of the injectable form, this would be another option for you.
• There has been no proven benefit to using both the oral and injectable forms of these medications, so you would choose one or the other. You may not use the oral medication with injectable SEMA or TZ.
• This makes an excellent maintenance choice for patients transitioning from both injectable SEMA and TZ.
• This medication is best kept refrigerated. This helps ensure it stays effective for your treatment period.
  
• The sublingual delivery vehicle has advantages over a traditional pill that is dissolved in the stomach. The medication is absorbed directly into the bloodstream through the mucous membranes under the tongue. This bypasses the digestive system, which can help the medication work more quickly and may help decrease gastrointestinal side effects.
  
  
  
  
  
  
• The max dose of injectable TZis 15mg weekly. Less medication will be absorbed by the body taking it orally; therefore, the max dose of this oral form is 5mg a day (35mg a week). Studies show that 98% of the peptide is destroyed by stomach acid, but the sublingual (under the tongue) route helps you absorb the medication before it reaches the stomach. Also, the larger doses mentioned above are given to assure adequate absorption by the body.
• Although the process of compounding an oral medication may be less complicated and less expensive than compounding a sterile (injectable medication), the medication is still pricey due to the much larger amounts of tirzepatide needed in the oral medications to see results.
• I do allow some flexibility with your dosing, but please use each bottle within 90 days. Patients on this product must order at least every 80-90 days to stay compliant. Using medication longer than recommended can cause stalls due to the breakdown of the peptide over time, which results in lower doses delivered into your system.

DOSING AND ORDERING GUIDELINES:

There are three concentrations of the TZ liquid medication, 3mg/ml, 4mg/ml, and 5mg/ml. There are #30 day options and #90 day options.

1) For 30-day ordering, your dosing is 1ml a day and you use the corresponding concentration:

*3mg dose: 3mg/ml (30ml bottle)--Place 1ml SL one time daily for 30 days $275

*4mg dose: 4mg/ml (30ml bottle)--Place 1ml SL one time daily for 30 days $325

*5mg dose: 5mg/ml (30ml bottle)--Place 1ml SL one time daily for 30 days $375

2) There are two 90-day options. These both use the 5mg/ml concentration. Please note that since you are using the higher concentration bottle, your 3mg and 4mg dose will NOT be 1ml with this bottle! Please do not use your bottle for longer than 90 days.

*Initial 90 day titration (75ml): Use 0.6ml (3mg) daily X 30 days, 0.8ml (4mg) daily X 30 days, then 1ml (5mg) daily X 30 days $850

*Maintenance Dose 5mg Daily (90ml): Use 1ml (5mg) daily X 90 days

3) For patients with sensitive stomachs, I'd recommend using each dose for at least 30 days before moving up. The individual #30 day supplies of the lower doses would be best for you. This is the standard dosing.

4) For new patients that want the option to move their dose up more quickly, I will allow alternative dosing. In this case, you would order the #90 day initial titration listed above. As long as you use the 3mg and 4mg doses for at least 10 days, you may move up more quickly. Your bottle of course will not last the full 90 days since you are escalating more quickly, but it's a great option for those who do not want to use the lower doses for a full 30 days each.

5) Patients that have previously been on TZ or SEMA at any dose in the past would definitely benefit from moving up more quickly. If it's been more than 4 weeks since your last dose, I'd recommend #4 above. If it's been within 4 weeks, see #6 below.

6) If you have been on TZ or SEMA (at any dose) within the past 4 weeks, you would see the best results starting at the 5mg dose. You may order a #30-day or #90-day supply.

*Most patients end up taking 5mg daily. A 30-day supply for this is $375. A 90-day supply is $950 (about $316 for a 30-day supply)*
