Sema/B6 SC Injection with E-Visit REVIVE RX

This service request will not be completed until all enrollment steps have been taken. New patients will receive an “Approval Message” through your Push Health account when you are approved for the program and another message once your service has been competed. Returning patients may return and place a new order request when ready and do not need prior approval. 1. This product ships to you from REVIVE RX PHARMACY. The medication will need to be refrigerated after arrival, so please place it in the refrigerator when you receive it. Please allow 7-10 business days for shipping.
2. Injection supplies are included. It will need to be kept refrigerated.
3. You must order the dose you intend to take! If you need to alter your dose in any way, please discuss this with our team by reaching out through your Push Health account.
4. After the 7-week starter package, you may skip the 4th week using the 0.5mg dose and go straight to the 1mg dose if you are doing well with minimal side effects. Or, you may stay on the 0.5mg dose with the next order.
   
5. These vials come in three different concentrations. Please make sure to note that your dose changes when you change strengths.
   
   
   1. TZ/B6 2.5mg/2mg/ml (1ml, 2ml, and 3ml vials): You will use these vials for the 0.25mg, 0.5mg, and 1.75mg doses.
   2. 3mg/2mg/ml (4ml vial): This vial has 12mg total. The most efficient use of this vial is to inject 2mg (66 units) weekly X 6 weeks.
   3. 5mg/2mg/ml (2ml vial): You will use these vials for the 2.5mg dose.
6. The BUD on these vials is 90 days, which translates to 75-85 days after you receive it. Some of these packages may be stretched out to every 10-14 days, but please do not use your vial past the BUD. The 4-5 week packages make great options for extending your dosing out.

• 0.5mg = 20 units/0.2ml
• 1mg = 40 units/0.4ml
• 1.75mg = 70 units/0.7ml*
