You are requesting a service from Dr. K, which includes a chart review for eligibility. If approved, the listed item above will be shipped to the specified address on the order. Because this requires a prescription, you must be a registered patient under Dr. K on Push Health. If you have not registered, please click HERE to get started. Unless otherwise approved, new patients must request a 0.25mg dose package or lower of SEMA.

This service request will not be completed until all enrollment steps have been taken. New patients will receive an “Approval Message” through your Push Health account when you are approved for the program and another message once your service has been competed. Returning patients may return and place a new order request when ready and do not need prior approval. Please message us through your Push Health account with any questions before requesting a service.

1. This SEMA is a clean version with no additives.
2. You will receive ONE vial for each package ordered. Injection supplies are included.
3. This product will be shipped to you from KADUCEUS PHARMACY. The BUD will be around 75-80 days once received. Packages and order guidelines assure you will be able to use all of your medication before the BUD.
4. For 4 and 5-week packages, you may order one or two packages at a time. For 6+ week packages, you may order only one package at a time. Those injecting every 7-10 days should order 4-week packages only. Please scroll down to the bottom to review the JC statement on recommended vial use.
5. Please allow 5-7 business days for shipping. It will arrive to you cool and will need to be kept in the refrigerator.
6. Please order the dose you plan to take. If you need to alter your dosing from what is on your packaging, please message us through your patient portal.
7. After the 7-week starter package, you may skip the 4th week using the 0.5mg dose and go straight to the 1mg dose if you are doing well with minimal side effects. Or, you may stay on the 0.5mg dose with the next order.
8. Please note that some doses of Sema products from Kaduceus with no additives use different vial sizes and concentrations than the Kaduceus products with additives. Please refer to instructions on your package to assure proper dosing.
9. There are TWO DIFFERENT vial concentrations for this product.
   
   1. The 0.25mg, 0.5mg,1mg, and 1.75mg packages use the 2.5mg/ml concentration.
   2. The 2.5mg dose uses the 5mg/ml concentration

DOSING

Sema Standard 2.5mg/ml

• 0.25mg: 10units (0.1ml)
• 0.5mg: 20units (0.2ml)
• 1mg: 40units (0.4ml)
• 1.75mg: 70units (0.7ml)
  
  

Sema Standard 5mg/ml

• 2.5mg: 50 units (0.5ml)
  
  

New patients not previously on an injectable weight loss medication will start at the 0.25mg for the first 4 weeks. You may then move up to the 0.5mg dose for the next 4 weeks, then 1mg for 4 weeks, etc. However, you do NOT have to move up in dosing until you are ready. If you are having successful weight loss on a certain dose, you do not have to move up. Also, if you are having significant side effects on a dose, you should spend another 4 weeks on your current dose before you move up.

Joint Commission guidelines for proper use of many multi-dose vials include the recommendation to discard the vial 4 weeks after first puncture, even though the Beyond Use Date (BUD) may be longer. These guidelines are generally followed in office and hospital settings, particularly when the vial is used for multiple different patients. However, this same suggestion is made for vials punctured several times a day or only once a week. With certain vial sizes/doses/packages, using all of the medication in the vial without wasting any could result in a patient using the vial for longer than 28 days after first puncture. If you choose to use your vial longer than 28 days after first puncture due to package size or maintenance dosing, you must acknowledge that you understand the Joint Commission recommendations to discard the vial 28 days after first puncture, even if you only puncture the vial once a week. You will be using the vial at your own discretion.
