1. This product is manufactured by PERFECT RX PHARMACY.
2. We use a partner platform, VITLRx, to place and handle your orders from Hallandale. VITLRx has a portal for patients to view their orders and receive tracking information. You will receive an email directly from VITLRx with instructions on setting up and accessing your portal. You may also sign up for shipping notifications from UPS at www.ups.com and www.fedex.com.
3. This TZ is compounded with B12. The concentration of B12 is 500mcg/ml. Each 50 unit injection will contain 250mcg of B12. The B12 will make the liquid have a pink or reddish tint to it. This is normal.
4. Injection supplies are included.
5. Each vial contains 4 weeks of your dose. Your dose will always be 50 units unless otherwise directed.
6. Please allow 7 business days for your order to be shipped to you. Creating your portal with them is the best way to track your package.
7. It will arrive to you cool and will need to be kept in the refrigerator.
8. You must order the dose you intend to take! If you need to alter your dose in any way, please discuss this with our team by reaching out through your Push Health account.
9. Patients in the weight loss phase should be injecting weekly for best results. For patients in maintenance, you may inject every 10-14 days with these since the BUD is longer.
10. If you are injecting weekly, you may order two vials at a time, keeping the above BUD in mind.

DOSING: ALWAYS INJECT 50 UNITS UNLESS INSTRUCTED OTHWISE.

New patients not previously on an injectable weight loss medication will start at the 2.25mg for the first 4 weeks (from another pharmacy). You may then move up to the 4.5mg dose for the next 4 weeks, then 6/75mg for 4 weeks, etc. However, you do NOT have to move up in dosing until you are ready. If you are having successful weight loss on a certain dose, you do not have to move up. Also, if you are having significant side effects on a dose, you should spend another 4 weeks on your current dose before you move up.

Joint Commission guidelines for proper use of many multi-dose vials include the recommendation to discard the vial 4 weeks after first puncture, even though the Beyond Use Date (BUD) may be longer. These guidelines are generally followed in office and hospital settings, particularly when the vial is used for multiple different patients. However, this same suggestion is made for vials punctured several times a day or only once a week. With certain vial sizes/doses/packages, using all of the medication in the vial without wasting any could result in a patient using the vial for longer than 28 days after first puncture. If you choose to use your vial longer than 28 days after first puncture due to package size or maintenance dosing, you must acknowledge that you understand the Joint Commission recommendations to discard the vial 28 days after first puncture, even if you only puncture the vial once a week. You will be using the vial at your own discretion.
