You are requesting a service from Dr. K to provide the listed item above to be shipped to the specified address on the order. Because this requires a prescription, you must have received an *Approval Message* from Dr. K to your Push Health account prior to requesting this service. You also must also pay your New Patient Fee if applicable. Do not request this service without prior approval or your request may be refunded minus a 5% processing fee. Returning clients may request a service when ready and do not need permission to request a new service. SHIPS TO ALL MY STATES

1. This TZ is compounded with Glycine
2. You will receive ONE vial for each package ordered. Injection supplies are included.
3. This product will be shipped to you from KADUCEUS PHARMACY. The BUD will be around 75-80 days once received. Packages and order guidelines assure you will be able to use all of your medication before the BUD.
4. For 4-week packages, you may order one or two packages at a time. For 6+ week packages, you may order only one package at a time. Those injecting every 7-10 days should order 4-week packages only. Please scroll down to the bottom to review the JC statement on recommended vial use.
5. Injection supplies are included.
6. Please allow 5-7 business days for shipping. It will arrive to you cool and will need to be kept in the refrigerator.
7. Please order the dose you plan to take. If you need to alter your dosing from what is on your packaging, please message us through your patient portal.
8. Please note that TZ products from Kaduceus with no additives use different vial sizes and concentrations than the Kaduceus products with additives. Please refer to instructions on your package to assure proper dosing.
9. There are TWO DIFFERENT vial concentrations for this product.
   
   1. The 2.5mg, 5mg, and 10mg 4-week packages use the 10mg/ml concentration.
   2. The 10mg X 6 weeks, 12mg X 5 weeks, and 15mg X 4 weeks packages all use the same 60mg vial, which has a 20mg/ml concentration.
   3. Therefore, please note that the 10mg dose will either be 100 units or 50 units, depending on your vial and package.
10. The 12.5mg dose is best given as a 12mg dose X 5 weeks to maximize the usage of your vial.

DOSING

While these are the dosages and vial concentrations used for the packages, please see chart for any approved dosage adjustments.

TZ 10mg/ml vial:

2.5mg = 25 units

5mg = 50 units

7.5mg = 75 units

10mg = 100 units

TZ 20mg/ml vial:

7.5mg = 37.5 units

10mg = 50 units

12mg = 60 units

15mg = 75 units

*These HIGHLIGHTED dose directions will be used on your packaging. Please note that the amount you inject for the 10mg dose will differ depending on package.

New patients not previously on an injectable weight loss medication will start at the 2.5mg for the first 4 weeks. You may then move up to the 5mg dose for the next 4 weeks, then 7.5mg for 4 weeks, etc. However, you do NOT have to move up in dosing until you are ready. If you are having successful weight loss on a certain dose, you do not have to move up. Also, if you are having significant side effects on a dose, you should spend another 4 weeks on your current dose before you move up.

Joint Commission guidelines for proper use of many multi-dose vials include the recommendation to discard the vial 4 weeks after first puncture, even though the Beyond Use Date (BUD) may be longer. These guidelines are generally followed in office and hospital settings, particularly when the vial is used for multiple different patients. However, this same suggestion is made for vials punctured several times a day or only once a week. With certain vial sizes/doses/packages, using all of the medication in the vial without wasting any could result in a patient using the vial for longer than 28 days after first puncture. If you choose to use your vial longer than 28 days after first puncture due to package size or maintenance dosing, you must acknowledge that you understand the Joint Commission recommendations to discard the vial 28 days after first puncture, even if you only puncture the vial once a week. You will be using the vial at your own discretion.
