You are requesting a service from Dr. K, which includes a chart review for eligibility. If approved, the listed item above will be shipped to the specified address on the order. Because this requires a prescription, you must be a registered patient under Dr. K on Push Health. If you have not registered, please click HERE to get started. Unless otherwise approved, new patients must request a 2.5mg dose package or lower of TZ.

This service request will not be completed until all enrollment steps have been taken. New patients will receive an “Approval Message” through your Push Health account when you are approved for the program and another message once your service has been competed. Returning patients may return and place a new order request when ready and do not need prior approval. Please message us through your Push Health account with any questions before requesting a service.

  1. This medication is shipped from REVIVE RX PHARMACY. It will arrive cool via overnight shipping and will need to be refrigerated upon arrival. Current shipping time is around 5-7 business days. Supplies are included.
  2. Please follow dosing instructions on your vial, as the concentration changes as you move up in dosing. For instance, the 5mg and 10mg dose are both 50 units because the concentration is different for each vial.
  3. Also, please note that that the 12mg and 15mg doses use the SAME vial, but you inject 60 units versus 75 units. I chose the 12mg instead of 12.5mg because you will be able to inject 5 weeks of the 12mg dose.
  4. ALSO, please note that there are 2 vials that are used for the 15mg dose. One offers 4 weeks of medication (60mg total). The other is a supersized vial (90mg total) with 6 weeks worth of medication. The 90mg supersized vial is MORE CONCENTRATED; therefore, you only inject 50 units (as compared to 75 units with the vial that is shared with the 12mg dose). Please refer to the JC guidelines for using a vial past 28 days below.
  5. The supersized vial MAY ONLY BE USED FOR INJECTING THE 15MG DOSE. No exceptions!
  6. The BUD on these product is 90 days (generally 75-80 days after you receive it). Therefore, you may order ONE or TWO vials at a time, EXCEPT for the supersized vial. You may only order ONE vial of the 90mg vial due to the BUD.


New patients not previously on an injectable weight loss medication will start at the 2.5mg for the first 4 weeks. You may then move up to the 5mg dose the next 4-weeks, then 7.5mg for 4 weeks, etc. However, you do NOT have to move up in dosing until you are ready. If you are having successful weight loss on a certain dose, you do not have to move up. Also, if you are having significant side effects on a dose, you should spend another 4 weeks on your current dose before you move up.



Joint Commission guidelines for proper use of many multi-dose vials include the recommendation to discard the vial 4 weeks after first puncture, even though the Beyond Use Date (BUD) may be longer. These guidelines are generally followed in office and hospital settings, particularly when the vial is used for multiple different patients. However, this same suggestion is made for vials punctured several times a day or only once a week. With certain vial sizes/doses/packages, using all of the medication in the vial without wasting any could result in a patient using the vial for longer than 28 days after first puncture. If you choose to use your vial longer than 28 days after first puncture due to package size or maintenance dosing, you must acknowledge that you understand the Joint Commission recommendations to discard the vial 28 days after first puncture, even if you only puncture the vial once a week. You will be using the vial at your own discretion.

If you have any concerns about this issue and for best safety practices, please choose a 4-week package and discard any medication leftover 28 days after first puncture.
